DATA OWNER APPLICATION FORM
1. General
This Application Form has been prepared by ION Tekstil LTD.. (“The Company”) who act as data controller in order to be able to evaluate and analyse in an effective and comprehensive manner, the applications which will be made by your end, data owners, in accordance with Articles 11 and 13 of the Personal Data Protection Law numbered 6698 (“KVKK”).
2. Application Method
You, data owners, can communicate your requests for enforcement of KVKK in writing to our Company with the capacity of a data manager under articles 11 and 13 of the KVKK by either completing this form or other methods to be determined by the Personal Data Protection Board (the “Board”):
- By personally applying to the address Halkalı Merkez Mahallesi, No:56/2, 34303 K.çekmece/İstanbul/Türkiye with a original signed copy of this data owner application form,
- By sending a original signed copy of this data owner application form to the addressHalkalı Merkez Mahallesi, No:56/2, 34303 K.çekmece/İstanbul/Türkiye with registered letter with return receipt or through a notary public,
- By sending an electronically signed copy of this data owner application form to the email
address info@iontekstil.com
It is required to submit a notarised power of attorney together with this Form in case applications are made by a third person on behalf of the data owner and as for applications made on behalf of children under custody/guardianship, the documents certifying the custody/guardianship relation should be submitted together with this Form.
In accordance with Article 13 of the KVKK, the Company acting in the capacity of data controller, shall respond to written access request of the data subject within the shortest time possible depending on the content of the request and within thirty (30) days at the latest. Your requests will be concluded by Our Company complimentarily and in case the responding process causes a cost, a fee may be charged as set out in the applicable legislation.
The Company may communicate with you and request some information and documents to confirm that you are the owner of the data. In this regard, the information and documents that you have provided us with will be destroyed immediately upon confirmation of your ownership of data.
In case the requested information and documents are incomplete, upon our request, the information and documents must be completed and communicated to us. The term of thirty (30) days provided under article 13/2 of the KVKK for the finalization of such request shall be suspended until such information and documentation are completely delivered to us.

Your personal data provided to us above is obtained in order to evaluate, finalize this form and contact with you and shall not be subject to data processing for any other purposes.
4. Data Owner's Requests

This application form has been provided to enable us to identify your relationship with our Company; to determine your personal data that has been processed if such a relationship exists; and to respond to your application accurately and within the legally-defined timeframe. To eliminate any legal risks that may stem from the illegal and unfair data sharing and to ensure the safety of your personal data, our Company reserves the right to request additional documents and information to verify your identity, and to examine whether you are authorized to make such an application. Should the information accompanying the requests communicated in this form prove to be inaccurate or not up-to-date, or in should it be ascertained that the application has been submitted by an unauthorized applicant, our Company will decline any responsibility related to an application containing false information or made by an unauthorized applicant.
5 - Data Owner's Statement
